| Disbursement Request Form | This form should be used to make new, one-time funding requests. The Executive Board will consider requests that are in support of projects and events that benefit Ottawa Hills Schools. |
| Reimbursement Request Form | This form should be submitted to the OHSPA Treasurer to receive reimbursement for an approved OHSPA-related expense & it must be accompanied by a receipt. |
| Tax Exempt Certificate | This form can be provided to vendors when buying something on behalf of an OHSPA-approved event to receive tax-exempt purchasing. |
| W-9 | This form can be provided to donors upon request by the OHSPA Treasurer. |
